Overview

Location: Family Practice Clinics - Naval Medical Center Portsmouth, VA

 

Hours of Performance: Services shall be provided Monday through Friday, between 0700 and 1600

 

Pay: $26.26/hr, Health & Wellness $5.06*/hr worked (in lieu of benefits)

Responsibilities

  • Provide patient care in accordance with Standards of Nursing Care, clinic policies, and provider orders.

  • Check in and triage patients, obtain and document medical/family history, vital signs, and observations.

  • Order, collect, label, and track specimens/labs; follow up on results and notify patients per policies.

  • Assist physicians with exams, procedures, and treatments; prepare, position, and monitor patients.

  • Perform treatments: dressings, IV therapy, catheterization, suctioning, NG tube insertion, medication and immunization administration, and emergency response.

  • Prepare patients for tests, procedures, surgery, and follow-up visits; ensure diagnostic/therapeutic services and referrals are arranged.

  • Provide routine nursing care: hygiene, feeding, ambulation, and patient transport.

  • Use MHS GENESIS and other IT tools (CarePoint, ICDB, Secure Messaging) for documentation, patient screening, population health metrics, and proactive care management.

  • Track HEDIS measures, health promotion data, and maintain/update patient registries.

  • Orient new patients/families to clinic policies; educate on procedures, medications, equipment use, and follow-up care.

  • Support admission, transfer, and discharge planning; participate in team huddles and patient education planning.

  • Operate and maintain clinical equipment; ensure supplies and emergency equipment are available and functional.

  • Document and report patient symptoms, reactions, treatments, and care provided.

  • Handle patient communications (in-person, phone, secure messaging) with courtesy and confidentiality.

  • Participate in peer review, staff training, and support accreditation/compliance activities (NCQA, Joint Commission, IG).

  • Provide emotional support and promote patient/family understanding of care plans.

Qualifications

  • Education: Graduate from an accredited community college, junior college, college, university or vocational nursing program approved by the appropriate State agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The National League for Nursing Accrediting Commission (NLNAC) or the Commission on Collegiate Nursing Education (CCNE). As an alternative to the education requirements in the basic contract, the HCW may be a graduate of a State Board of Nursing approved LPN/LVN training program OR be a graduate of an accredited LPN program by Accreditation Commission for Education in Nursing (ACEN) OR have completed an equivalent military training program.
  • Experience: Possess a minimum of two (2) years of experience in a hospital, outpatient clinic, or acute care setting.
  • Licensure: Current, full, active and unrestricted license as a practical or vocational nurse.
  • Certifications: Basic Life Support (BLS)
