Position Title: Registered Nurse (RN) - Wound Care

Specific Department: Medical Nursing

Responsibilities

  • Coordinate patient care in collaboration with a wide array of healthcare professionals. Facilitate the achievement of optimal outcomes in relation to clinical care, quality and cost effectiveness.
  • Ensure compliance with standards of care and practice in accordance with all established policies, procedures, and guidelines used in the MTF.
  • Perform physical exam and health histories.
  • Provide health promotions, counseling, and education.
  • Administer medications, wound care, and numerous other personalized interventions.
  • Clinically direct and supervise care provide by other healthcare professionals.
  • Accountable for making patient care assignments based on the scope of practice and skill level of assigned personnel.
  • Recognize adverse signs and symptoms and quickly react in emergency situations.
  • Communicate and collaborate with a diverse group of people for the purpose of informing the healthcare team of plans/actions, for teaching/education to benefit the patient/family and organization.
  • Make referral appointments and arrange specialty care as appropriate.
  • Perform nursing services identified in the TO.
  • Conduct research in support of improved practice and patient outcomes.
  • Manages the ostomy/wound program for the medical center.
  • Develops policies, procedures, and protocols for the care of the ostomy wound patient. Coordinates, plans, directs, and evaluates the work of the multidisciplinary health care members involved in the care of the enterostomal patient.
  • Assist nursing staff in recognizing risk factors and initial preventive measures through policy development and formal/informal in-services.
  • Consults with physicians and nursing personnel on recommendations for the development of care plans for the patient and use of pressure reducing devices.
  • Develops and coordinates education programs for hospital personnel, patients, and families relevant to the management of ostomy and skin care problems.
  • Liaises with hospital supply companies and local ostomy associations to stay abreast of latest appliances, products, and techniques of care.
  • Acts as Enterostomal Therapist in both inpatient and outpatient settings.
  • Responds to requests for nurse consultant services.
  • Provides acute rehabilitative care for people with select disorders of the gastrointestinal, genitourinary and integumentary systems.
  • Provides preoperative and postoperative counseling, to include identification of the site of the ostomy, expected results of the surgery, dealing with the stoma, family counseling and adjustments, and providing outpatient clinical care.
  • Prior to surgery explains the operative procedure and expectations, discusses the disease process and explains and teaches the function of ostomy.
  • Visits postoperative ostomy patients, evaluates process of patient and family in acceptance of ostomate, and recommends necessary therapy.
  • Fits patients with ostomy appliance and teaches patients evaluation order control.
  • Regularly consults with patient’s physician to discuss progress and plan and care.
  • Instructs patient in appliance wear, irregularities of appliance fitting, supply, recording, skin care and medications used to maintain a health skin texture.
  • Teaches patients and families about potential skin problems and anticipated changes in regiment of care when ostomate is receiving chemotherapy and radiation therapy, dietary changes, and activity and other potential problems.
  • Provides sexual adjustment counseling, addresses patients concerns, acknowledges expressions and provides practical suggestions.
  • Maintains clinical records of patient treatment and care plans.
  • Evaluates patients at risk for skin breakdown with recommendations for preventative measures.
  • Enlists cooperation of nursing staff to individualize patient care and familiarize the staff with new innovations in ostomy and skin care supplies and procedures and in reinforcing emotional support to lessen the impact of hospitalization for the new ostomate.
  • Identifies training need and provides reliable related in-service training to staff.
  • Hyperbaric Medicine Clinic. The Registered Nurse (Wound Care) may be required to periodically work in the Hyperbaric Medicine Clinic, assist in outpatient wound care, and serve as an Inside Attendant (IA) in the multi-place hyperbaric chamber.
  • In order to serve as an IA, the nurse shall undergo a physical examination performed by a licensed provider in the Hyperbaric Medicine Clinic to ensure there are no contraindications to this duty.
  • Once the nurse has an approved physical, the clinic will conduct a test of pressure in the Hyperbaric Chamber which will consist of pressurization in the chamber to 2.8 atmospheres absolute or the equivalent of 60 feet of sea water.
  • In order to continue serving as an IA, the nurse must maintain medical standards for hyperbaric chamber duty and advise a physician in the clinic of any changes to his/her medical status. Prior to performance of IA duties, the nurse will receive approximately 8 hours of academic and hands-on training provided by the Hyperbaric Medicine staff.
  • During the performance of IA duties, the nurse will monitor patients throughout their treatment. The attendant will assist patients with equalizing pressure in their ears during pressurization of the chamber, will assist with donning of oxygen hoods/masks, and will respond to medical and non-medical needs that may arise while the patients are inside the chamber.

Qualifications

Degree/Education: Associates Degree of Nursing. Certifications in addition to Basic Life Support (5.2.3): None.

 

Experience: One year of experience within the last two years as a registered nurse. Knowledge of and ability to perform professional nursing practices and procedures, specializing in the treatment of enterostomal patients to include skill to manage all aspects of enterostomal therapy. Ability to coordinate, direct and evaluate work of multidisciplinary team involved in the care of enterostomal patients. Skill in synthesizing patient information (diagnostic/therapeutic tests and procedures). Knowledge of pharmaceuticals, their indications, interactions, contraindications and effects. Knowledge of emergency medical procedures and ability to respond appropriately. Knowledge and ability to use specialized equipment. Knowledge and ability to perform infection control procedures.

 

Licensure/Registration: Current, full, active, and unrestricted license to practice as a Registered Nurse.
