Position Title: Registered Nurse - Case Manager
Specific Department: Medical Group (MDG)
Schedule: Monday – Friday, 0730 – 1600

Participate in all phases of the Case Management Program (CMP), ensuring alignment with DoD, DHA, AF, and CM standards of care.
Provide nursing expertise across the CM process including assessment, planning, implementation, coordination, monitoring, and evaluation.
Develop and implement patient care strategies using inpatient, outpatient, onsite, and telephonic case management; integrate CM with utilization and social work management.
Identify patients for case management through multiple referral sources, conducting comprehensive assessments and developing individualized care plans with measurable short- and long-term goals.
Coordinate and advocate for patient care across military and community healthcare systems; ensure continuity of care through effective discharge planning, warm handoffs, and follow-up.
Utilize and maintain CM tools and databases, including clinical guidelines, patient identification and assessment tools, and community resource directories.
Serve as a liaison and advocate between patients, families/caregivers, healthcare providers, and community agencies; promote education and adherence to care plans.
Ensure patient education based on assessed learning needs, including disease processes, treatments, and healthcare system navigation.
Document and update treatment plans, ensuring accuracy and compliance with HIPAA, TJC, AAAHC, and other applicable accreditation and regulatory standards.
Monitor and evaluate care outcomes, patient adherence, resource utilization, and case management effectiveness using established metrics.
Participate in peer reviews, case reviews, quality improvement initiatives, and multidisciplinary meetings to support collaborative care.
Maintain liaison with appropriate internal and external agencies to facilitate safe, accessible, and effective care environments.
Provide support across various MTF areas including disease management, acute, ambulatory, and specialty care settings.
Travel as required for home inspections and patient support, in accordance with contract guidelines (Para 2.4.1).
Train and orient new staff; participate in facility committees, task forces, and case management working groups.
Ensure compliance with all applicable DoD, AF, DHA, MTF, and professional nursing policies, standards, and training requirements.
Maintain required documentation, coding, data tracking, and participate in ongoing education to support licensure and professional development.
Demonstrate strong communication, organizational, and problem-solving skills, with the ability to work independently and handle sensitive or complex cases.
Degree/Education: An Associate’s Degree in Nursing is required. Must be a graduate of an accredited professional nursing educational program.
Certifications (in addition to Basic Life Support): Must possess one of the following certifications:
Commission for Case Manager Certification: Certified Case Manager (CCM)
Certification of Disability Management Specialists Commission: Certified Disability Management Specialist (CDMS)
Association of Rehabilitation Nurses: Certified Rehabilitation Registered Nurse (CRRN)
American Board for Occupational Health Nurses: Certified Occupational Health Nurse (COHN) or Certified Occupational Health Nurse-Specialist (COHN-S)
National Board for Certification in Continuity of Care: Advanced Certification in Continuity of Care (ACCC)
Commission on Rehabilitation Counselor Certification: Certified Rehabilitation Counselor (CRC)
American Nurses Credentialing Center: Nurse Case Manager (RN-NCM)
National Academy of Certified Care Managers: Care Manager Certified (CMC)
Experience: Must have a minimum of three (3) years of full-time experience within the past four (4) years as a Registered Nurse Case Manager.
Licensure/Registration: Must hold a current, full, active, and unrestricted license to practice as a Registered Nurse.
